Waiver:	TOURNAMENT RELEASE AND WAIVER: I understand the rules of the tournament and will abide by them. I understand that I am participating in a sport that has body contact and understand that injury can occur with my participation. I assume full responsibility for all of my actions during and connected to the above tournament. I understand the risk of competing in this form of Martial Arts competition and hereby release the event organizers and all of its employees and associates, tournament sponsors and the event facility, from any type of injury, loss, or death sustained while competing in this competition. I also state that I am in good physical condition and know of no reason why I cannot participate in this event. I have current and valid health insurance. In case of emergency, I hereby authorize any licensed medical personnel to perform any accepted medical procedure deemed necessary and I agree to bear the expense of any such treatment. I also agree that my attendance and/or performance at the tournament will be photographed, filmed, and/or taped and used exclusively by BJMUTA for promotional or commercial purposes and I waive any compensation thereof. I agree to not use ANY footage obtained at the event for commercial purposes without expressed, written consent from BJMUTA. I waive any claims and promise not to sue the event organizers and/or BJMUTA with respect to any loss incurred during or in connection with my participation in the event, any activities associated with the event and the conduct and management of the event (including anything that may result from the negligence of the event organizer). I further agree to hold harmless and indemnify the event organizer and/or BJMUTA from any claims brought against the event organizer and/or BJMUTA resulting from, arising out of or in any way associated with any loss. Prior to participating in the event, I shall have the right to inspect the facilities and equipment to be used and, if I discover any condition which I believe to be unsafe, I will immediately cause event officials to be notified of such condition and will not participate in the event so long as such condition exists. I hereby fully and forever release: (a) BJMUTA, its affiliated organization, and their successors, assigns, officers, directors, members, agents, employees, insurers, representatives and attorneys; and (b) all manufacturers, distributors, wholesalers, suppliers, and/or retailers of the BJMUTA facilities of the equipment that I may use in connection with any and all activities conducted and/or sponsored in whole or in part by the BJMUTA Group from any and all claims, actions, or causes of action, damages, demands, costs, expenses, losses, and attorney's fees of every kind and nature, which I have, or may claim to have arisen out of or are related to arising out of or resulting from the transmission of a communicable disease or failure to perform services which were intended to or assumed to prevent communicable disease or their transmission to others. I agree that the foregoing release shall be binding on my heirs, distributes, guardians, executors, administrators, successors in interest and legal representatives. It is my intention that this release will be effective to bar all claims, demands, controversies, causes of action, liabilities, costs, expenses, attorneys' fees, and damages of whatsoever character, nature and kind, known or unknown, suspected or unsuspected, arising out of or related to the Activities.
